Please consider adding a comments field for the attributes so when I use a form I would simply hover the mouse of the attribute and it would show a popup comment field.
Example:
Note using Form Test
Device - Switch Port
Device- Wall Jack
If I am in a form and hover the mouse over the attribute name "Device - Switch Port" I would see a popup comment ie: This is for the network switch port only, not the patch panel port"

Attribute Comments
I have slightly hard time to exactly grasp how your form looks. Is it a multi-select list? A screenshot would be a bit more illustrative, I think
In general like the idea. Although the 'comment' label being bit ambiguous, regarding its meaning:
A) remark expressing an opinion or reaction.
B) A piece of explanatory text
I would use it as explanatory text. So could also be called 'descriptive field'
I tend like it on various levels
* Section (what's the purpose; what does it contain
* The (custom) attribute as such; The label tend to be brief. How the attribute being used might be less self-explaining after 6 months later.
* The items in the single/ multi-select list (in case of some ambiguity)
* The custom typed attribute text (if some more extensive comment can be helpful)
