EListError when inserting document from file

Occasionally this error pops up when inserting a document from file. I have seen it in version 4.1 and now also in 4.2.

Error type: EListError
Message: List index out of bounds (68 )
ID 5543

After cancelling out of the error, if I try again, it works. So, it is more of an annoyance than an issue.

Does it happen with particular file type (Ms Word, Rich Text Format, plain text) or it happens with any file?

I think it has happened with both RTF and Word documents, but lately I have inserted both of those type of files, but it didn't happen. I'll keep an eye on it.
