In prior versions of MI, there was a button I could add to the toolbar that automatically created a clone of the selected document, without having to go to the clone box. I don't see it in MI6. Was it removed?

Yes, it was removed. You can now use Ctrl+C, Edit > Paste Special > Paste Clone. We will add keyboard shortcut for the latter, until then you can assign your own.
