Not sure why some of the entries are gray and the rest are not, thoughts?

If I'm not mistaken, it's usually gray when the "Note" is empty.

Thank you, anyway to turn this off, I use lots of forms and do not always have a need to use the notes, seeing the entry non-gray would be my preference at all times?

You need to disable the formatting rule for this in Format > Conditional Formatting > Empty Note. It is per-notebook setting.
